Please pick up the enrollment form from the office or the enrollment form can be downloaded from the website and completed with the computer. Please complete the form and bring it with you.

At the time of enrolment, please bring the following to the meeting:

• Filled in Enrolment Form
• Your child’s birth certificate or passport
• Immunisation certificate or Well Child booklet
• Photo ID for the parent / guardian signing the Enrolment Form
